Annual Registration - 2018-2019

Information and Documents 

To ensure continued enrollment, all families must attend Registration Day in August (date TBD) to complete paperwork, pay student fees, and receive class placement information.  Families unable to attend Registration Day may submit the completed packet by postal mail to FHM, or may bring the completed packet to "Registration Make-up Day."

Please note that FHM is required by Jeffco Schools and CDHS Child Care Licensing to collect all forms annually.  Students not submitting required documents by the date due will not be permitted to attend until such forms are submitted and accepted by the front office.  Paper copies of all items, except those in Jeffco Connect, are required.  Please do not email or fax your child's records.

Below is a list and chart of forms that were required for the 2017-2018 school year.  An annual statement of health and immunization records will be due at registration for the 2018-2019 school year.  The table below is a sample of our annual registration process and this will be updated near the end of the school year.

  • Meet a CSN representative
  • Learn about volunteering opportunities & how to fulfill 30 volunteer hours this year!
Jeffco Connect
  • Log into Jeffco Connect
  • Update student and family information (including address and contact info)
  • Review directory information policy
  • Review and acknowledge Jeffco’s Conduct Code (must be dated 7/27/17 or later)
  • Select “no” to Jeffco’s “Bring Your Own Device” policy (must be dated 7/27/17 or later)
  • Update Significant Health Concerns with any and all known health concerns

Student Fees
  • See the 2017-2018 Fee Schedule, 2018-2019 Fee Schedule
  • (Not Available until July 31, 2017) Payment via Jeffco Connect is the much preferred method.  Please print your receipt or payment history for confirmation of payment at registration!
  • Fees may also be paid by check, credit card or cash (exact change ONLY) to the Financial Secretary

*Note: Health Statements submitted after January 1st, 2017 may still be in effect and it may not be necessary to submit an additional one. Information regarding Health Statements will only be available at Registration Day for those attending in person.
